Transition Services
Under the IDEA (2004), by the age of 16, students are required to have an Individualized Transition Plan (ITP) as part of their IEP. Transition services are activities that prepare students with disabilities to move from school to post-school life. The activities must be based on the students’ needs, preferences, and interests, and shall include needed activities in the following areas:
